Homeland Palliative Care


Comforting those with serious illness

Homeland’s Palliative Care team helps patients and their families have a better quality of life by providing comfort and relief from the symptoms and stress of serious illness. Palliative care also helps families establish goals of care and assists with advanced care planning.

Homeland provides community-based palliative care anywhere you call home. Our Palliative Care team consists of board-certified nurse practitioners, a licensed medical social worker and a physician medical director.

Is palliative care right for you or your loved one?

Palliative care may be right if you or your loved one suffers from pain, stress or other symptoms due to a serious illness. These may include cancer, chronic obstructive pulmonary disease (COPD), congestive heart failure (CHF), liver disease, kidney disease, Parkinson’s, amyotrophic lateral sclerosis (ALS), dementia, stroke, HIV/Aids and other serious illnesses.

Palliative care is based on the needs of the patient, not on a specific diagnosis. It is appropriate at any stage in a serious illness and can be provided along with curative treatment. Homeland’s palliative care program cares for patients who are 18 years and older.

What can I expect from palliative care?

The goal of palliative care is to reduce and eliminate, where possible, symptoms such as pain, fatigue, depression, anxiety, difficulty sleeping, shortness of breath, constipation, nausea, vomiting and loss of appetite. Palliative care can help you carry on with your daily life and improve your ability to go through medical treatments. Discussions about advance care planning also can help you and your family with choices for care that are consistent with your goals, wishes and priorities.

Does insurance cover palliative care?

Homeland at Home’s palliative care services is currently available through Medicare coverage. We hope to develop partnerships through additional insurance programs in the future.

How do I request a palliative care consult?

Tell your doctor or nurse you would like to see a palliative care team. A written order from your healthcare provider will be needed. Our team will work in partnership with your healthcare providers to make sure that you receive the best care possible.
