What is Homeland at Home?
Homeland at Home, a community outreach program of Homeland Center, provides a full continuum of in-home services to care for patients and to support families as their needs change.
Services include hospice care (end-of-life care for patients and support for families); home health (skilled and therapeutic physician-ordered services); and home care (daily living assistance and companionship for seniors).
By providing compassionate, in-home care, we keep your loved one near to make the most of every precious moment together.

What is Homeland Center?
Chartered in 1867, Homeland Center is a licensed not-for-profit Continuing Care Retirement Community offering exceptional personal and skilled nursing care. Homeland Center occupies a full block in uptown Harrisburg, which includes a 24-bed unit dedicated to the special needs of those with Alzheimer’s or dementia-related diseases. Homeland’s Five-Star Skilled Nursing Care Facility designation – Medicare’s highest recognition for quality care, staffing, and safety – ranks it among the country’s best.
Who is eligible for Homeland at Home services?
Eligibility varies by type of service.
- A hospice evaluation can be requested when a patient is diagnosed with a life-limiting illness.
- Home health services require a physician-ordered home medical treatment plan.
- Home care services are available for anyone in need of in-home daily living assistance.
- Bereavement support is available to anyone in the community who is experiencing grief.

Does Homeland at Home accept insurance?
Homeland at Home accepts Medicare, Medicaid and long-term insurance. Please contact us to confirm if your insurance is accepted for the particular service needed or if other arrangements are available.
